Here you can find and download forms for your teen. MEDICAL RELEASE FORMS are required for your teen to participate in any event.

EPYM Policy Form

This code of conduct is a list of requirements that youth group members must follow when participating in youth activities. Please have your student read over and sign and return to the Youth Minister.

EPYM Medical Release Form

This Medical Release Form form gives the Youth Minister and other chaperones important information regarding your teen’s medical condition. Please list all need to know information as well as medication. This is a requirement to participate in any youth event where the parent is not present. EPCOC needs an updated copy every year.

Scholarship Application

We would love to have your teen involved in events and camps. If for whatever reason you are not financially able to afford for your teen to participate please let the Youth Minister know and fill out this Scholarship Application form. EPCOC will try their best to scholarship your teen for the event.
